Understanding Acute Kidney Injury (AKI): Causes, Symptoms & Prevention

Acute Kidney Injury (AKI) refers to a sudden, rapid decline in renal function occurring over hours or days. When AKI strikes, the kidneys lose their capacity to filter metabolic waste products, balance bodily fluids, and regulate critical electrolytes.
Because AKI can progress rapidly, prompt medical diagnosis and intervention are vital to prevent permanent renal tissue damage or life-threatening systemic complications.
The 3 Major Categories of Acute Kidney Injury
1. Prerenal AKI (Decreased Renal Blood Flow)
The most common form of AKI, occurring when blood flow to the kidneys is severely restricted without direct damage to the kidney tissue itself.
- Primary Causes: Severe dehydration (hypovolemia), massive blood loss (hemorrhage), acute heart failure, sepsis, or liver failure.
- Key Insight: Early fluid resuscitation and restoring blood volume often completely reverse prerenal AKI.
2. Intrinsic / Intrarenal AKI (Direct Kidney Tissue Damage)
Arises from direct structural injury to the glomeruli, renal tubules, blood vessels, or interstitium.
- Primary Causes: Acute Tubular Necrosis (ATN) from ischemia, nephrotoxic medications (NSAIDs, aminoglycosides), IV contrast dyes, glomerulonephritis, or systemic infections.
- Key Insight: Recovery requires specialized nephrology management to allow damaged renal cells time to heal.
3. Postrenal AKI (Urinary Tract Obstruction)
Occurs when physical blockages stop urine from exiting the body, creating backpressure that damages renal tissue.
- Primary Causes: Kidney stones, Benign Prostatic Hyperplasia (BPH), retroperitoneal tumors, or pelvic surgical scarring.
- Key Insight: Removing or bypassing the obstruction quickly restores normal renal clearance.
Common Symptoms of AKI
Although AKI can be asymptomatic in its initial stages, key warning signs include:
- Decreased or absent urine output (oliguria or anuria)
- Fluid retention causing swelling in the legs, ankles, or periorbital area
- Unexplained fatigue, persistent nausea, or vomiting
- Shortness of breath (due to fluid accumulation in the lungs)
- Confusion, drowsiness, or cardiac arrhythmias caused by high potassium (hyperkalemia)
