Swap Kidney Transplant

Swap Kidney Transplant (Paired Exchange)

A simple guide on how two mismatched families can safely and legally swap donors to save two lives.

What is a Swap Kidney Transplant?

Sometimes, a family member is completely healthy and wants to donate a kidney to their loved one, but they cannot do so because their blood groups do not match, or their tissue cross-match is positive. This is called an incompatible pair.

A Swap Kidney Transplant (also known as Paired Kidney Exchange) is a brilliant, legal solution for this problem. It connects your family with another family facing the exact same matching issue. By safely swapping donors between the two families, both patients receive a perfectly matching, healthy kidney.

Understanding the Swap with a Simple Example:

Family A: Patient A needs a kidney, but Donor A’s blood group does not match.

Family B: Patient B needs a kidney, but Donor B’s blood group does not match.

The Swap Solution:

Donor A gives their kidney to Patient B, and Donor B gives their kidney to Patient A. Both surgeries happen at the exact same time, and both families win!

The Swap Transplant Process Step-by-Step

1

Joining the Swap Registry

Your family’s blood group and medical data are entered into a highly secure registry at our transplant center. The database looks for another family that matches your requirements perfectly.

2

Legal and Authorization Approval

In India, all swap transplants are highly regulated to ensure complete safety and transparency. Under the Transplantation of Human Organs Act (THOA), a formal government-appointed authorization committee reviews and approves the files of both families.

3

Simultaneous Surgery

To avoid any change of heart or ethical issues, the operations for both pairs are performed at the exact same time in adjacent modular operation rooms at Medanta.

Major Benefits of choosing a Swap Transplant

  • Cost-Effective: Unlike an ABO Incompatible transplant, a swap transplant does not require expensive blood washing (plasmapheresis) sessions or high-dose injection therapies.
  • Natural Matching: Because the kidney you receive is a natural biological match, the risk of early organ rejection is much lower, and the kidney can last for a very long time.
  • Bypasses Waiting Lists: It severely cuts down your time on the deceased (cadaveric) waiting list, helping you get off dialysis much quicker.
