Desensitization Protocols for High Antibody Kidney Transplant

Desensitization Protocols for High-Risk Transplants

An easy patient guide on how we lower high antibody levels to turn a “mismatched tissue” into a safe transplant.

What is a “Sensitized” Patient or Positive Crossmatch?

Every human has an immune system that uses “antibodies” to protect the body from foreign invaders like viruses. However, if a kidney patient has had a previous kidney transplant, multiple blood transfusions, or multiple pregnancies, their body may have built up strong antibodies against other people’s tissues. These patients are called “highly sensitized.”

If we test your blood against your donor’s blood and find these tissue-fighting antibodies, it results in a Positive Crossmatch. A standard transplant cannot proceed under this condition because your body will attack the kidney immediately. **Desensitization** is a medical process that calms down your immune system and lowers these tissue antibodies so your body can accept the donor’s kidney safely.

How the Desensitization Process Works

1

Advanced Mapping (Luminex Test)

We perform a highly advanced blood test called a Luminex Single Antigen Bead assay. This test acts as a detailed map, pinpointing the exact strength and type of tissue antibodies inside your body so we can treat them precisely.

2

Filtering the Antibodies

Similar to blood group mismatches, we use advanced plasma filtration machines a few days before your surgery to actively wash out and reduce the harmful tissue-fighting antibodies from your bloodstream.

3

IVIg & Protective Medication Therapy

We deliver specialized liquid protein infusions called IVIg (Intravenous Immunoglobulin) alongside targeted medications. This therapy gently covers and silences any remaining antibodies, putting your immune system into a “sleep mode” so it leaves the new kidney alone.

Who needs a Desensitization Protocol?

This treatment track is highly specialized and is typically recommended for:

  • Patients seeking a second or third kidney transplant (Re-Transplants) who rejected a previous graft.
  • Patients whose crossmatch tests show positive reactions with an otherwise healthy family donor.
  • Patients with high **HLA (Human Leukocyte Antigen)** antibody score cards on their donor lists.

Excellent Long-Term Outcomes

While desensitization requires careful tracking and planning under an expert nephrologist, modern medicine makes it highly secure. Under the supervision of Dr. Debabrata Mukherjee at Medanta, our advanced immunomodulation treatments enable sensitized patients to achieve healthy kidney function and long-term survival rates comparable to routine transplants.
